Introduction
The Oregon Water Resources Department is pleased to announce online filing of well logs.  Using Adobe Acrobat Reader (a free download)  well logs can be filled out electronically, saved locally to the filers hard drive and submitted back to the Department as a completed log.   Below is a brief users guide to help you get started. 

Prerequisites
  1. Adobe Acrobat Reader (free version) version 7 must be installed. The forms will not work properly without this version of the software. You can download this free software at: http://www.acrobat.com

  2. Download form at:  http://www1.wrd.state.or.us/pdfs/MonitorForm.pdf

  3. Sign up with the Department for electronic filing and receive a password which will serve as your ‘electronic signature’ for online filing.

Working with the forms
Template well logs
If you have several wells constructed on one location, you can fill out one log and save it. Then you can copy that file to a new name, open it up, and change just those items that need to be changed.

Well logs can be filled out offline
Well logs can be filled out offline – you don’t have to be online to fill out the form, only to submit it. You can for example fill in the basic information and send the form out with the driller on a laptop. They could enter the information out in the field and it can be submitted once you are back in the office and hooked up to the Internet.

Saving the form locally
Unlike previous versions of the well log PDF, this version can be saved on your local machine with the free version of Adobe Acrobat. The department has purchased a license from Adobe which allows it to “reader extend” this form and make it so you can save the form as you fill it out.

Filling out the form
Owner (section 1)
Enter owner’s name and address as completely as possible

Type of work (section 2)
Check one. Describe alteration or abandonment procedures in the comments. For deepening indicate beginning and ending static water level under Section (10).

Drill methods (section 3)
Mark the correct box to indicate the drill method used.  

Construction (section 4)
Detail the characteristics of the well construction, including such things as:
  • Describe the monument/vault
  • Describe each change in borehole size or construction. 
  • Describe type of casing and liner and the locations. If a depth needs to be expressed as “above land surface”, check the box next to the “from depth” to indicate it is above land surface. 
  • Describe the type of screens installed, if any.
  • Describe the seal
  • Describe the filter

Well tests (section 5)
Mark the type of test that was used to determine the quantity of water available from the well. Be sure to indicate the temperature. The method that the strata were sealed should be indicated in Section 4 (bore hole construction).    
 

Location of well (section 6)
Indicate the location of the well as accurately as possible so that it can be located by other parties in the future. Indicate County, Township, Range, Section, and Tax Lot number, ¼ ¼ or latitude and longitude, and street address of the well or nearest address. If LORAN equipment is available, please report latitude and longitude in degrees, minutes, and seconds or decimals.           

Static water level (section 7)
Show the static water level of the completed well. Indicate the artesian pressure, if appropriate, be sure to click the check box to the left of static water level if it is above land surface. For deepening indicate beginning and ending static water level.     
 

Water bearing zones (section 7)
Indicate the depth at which water was first found. Show the water bearing strata and the position of the static water levels (SWL) – again, click the check box to the left if the static water level is above land surface. Include an estimate of flow rate, if possible.  

Well Log (section 8)
Describe the color, texture, grain size, and structure of materials and show the thickness of each stratum and aquifer drilled. There should be at least one entry for each change of formation.  Mark the dates and sign as appropriate at the bottom of the form.  

Overflow page (page 2)
This page is provided should there not be enough room on the first page for all of your entries in a section. Fill it out as appropriate. If you still cannot fit everything on the overflow page, you will not be able to submit electronically. You will need to print out the form and fill it out manually.
 

Map pages (pages 3 & 4)
The map pages allow you to insert a graphic showing the map of the well. Click on the blank map and you will be presented with a File browser to locate the file you have already created to insert. Graphic files in the .jpg format seem to work best for this.
 

How to submit the form
Electronic submittal
  1. Save the pre-submitted form – we recommend saving the form before submitting it to the Department. If an error occurs, you will still have a copy and not lose all your work. For example, you may save it as customer1_pre_submit.pdf

  2. Submit the form – once you are ready to file the report, click the Submit button. As part of the submittal process the form will be checked for obvious errors. Any errors will be highlighted in pink and will need to be fixed before the well log can be submitted. It will be looking for things such as:
    1. Land Owner filled out (first/last OR company name and address)
    2. Location of well
    3. Type of work
    4. Drill Method
    5. Bore hole entered, starting at zero, with no gaps in the from ~ to ranges.
    6. Seal method
    7. It will ensure that from/to depths are not greater than the max borehole depth
    8. Temperature
    9. Start Date, End Date
    10. And many more….

  3. Error checking at the server – once your form has passed the internal pre-checking, it will be uploaded to OWRD for processing. This program will run through more error checking. If  an error occurs at this point, you will receive a report back outlining all the problems that were encountered.  You will need to correct the errors and resubmit the well log. The program will look for errors such as:
    1. Do you have the right password for your license?
    2. Did this license number file the startcard?
    3. Does this license have the ability to file this type of well log?
    4. Has a well log already been filed on this startcard?

  4. Receiving the processed form – if no errors occur, you will receive an electronic  copy of the well log, stamped with the county code, well log number, and received date. This is your receipt that the log has been filed with the Department.  You will note that all the pages are numbered, blank pages are removed, and every page is stamped. This log is now available for viewing on the Department’s well log query page. You can now save this processed form for your records, print it out for your customers, etc.


Print and mail
While electronic submittal is not required it is strongly encouraged as a way to reduce processing costs. You can fill out the form, print it, and send it into the department to be processed as a normal well log. This report must be filed with the department within 30 days from the date of completion of well construction, deepening, alteration, abandonment, or conversion.
